Japanese encephalitis (JE) is a serious viral infection of the brain caused by the Japanese encephalitis virus and transmitted to humans through the bites of infected Culex mosquitoes. It occurs primarily in rural and agricultural areas of Asia and the Western Pacific, particularly in regions where rice paddies and pig farming coexist, as these environments support mosquito breeding and virus amplification. While the majority of infected individuals experience no symptoms or only mild illness, a small proportion develop severe encephalitis (inflammation of the brain), which can lead to permanent neurological damage or death.
Vaccination should be used alongside personal protective measures against mosquito bites. The JE mosquito is most active between dusk and dawn, so it is particularly important to use insect repellent containing DEET, wear long sleeves and trousers in the evenings, and sleep under treated mosquito nets during this time.
The vaccine (Ixiaro) is given as two doses administered 28 days apart. Both doses should be completed at least one week before potential exposure. The vaccine is suitable for adults and children from 2 months of age. A booster dose may be recommended for those with continued or repeated exposure risk.
The vaccine is administered as an injection into the upper arm. Common side effects include temporary soreness, redness, or swelling at the injection site, and occasionally mild headache, fatigue, or muscle aches. These are generally mild and resolve within a few days.
